I confirm that I am at least 18-years old. 

I understand that the personal information that I have given to the Reflexology Association of Canada (RAC) is for establishing and maintaining my membership with RAC and its Chapters. I understand that my membership information may be used to validate my membership status with key stakeholders: such as insurance providers. It may also be used to carry out RAC’s legal obligations under the Canadian Not-For-Profit Act.

 I understand that my information will not be used for any other purpose or shared with other parties without my express permission.

As a RAC member:

 I agree to promote and adhere to the principles and objectives of the Association as set forth in its By-Laws, Scope of Practice, Code of Conduct & Ethics, and Standards of Practice.

 I agree to act and express myself positively in accordance with RAC’s Values.

I understand that I can help shape the development of my Association to make reflexology provincially and federally recognized by getting involved locally, regionally, and nationally with the Association.

 I agree to pay a late fee of $15 (or the current late fee set by the Association) if I do not renew my membership within sixty (60) days of my invoice date.

 I also agree that if I do not renew my membership within six (6) months of my membership expiration date, my membership will lapse. At that time, to bring my membership to current status, I will have to pay a rejoining fee of $75 (or the current rejoin fee set by the Association), in addition to a prorated membership fee (plus GST/HST).

I understand and agree that membership fees are non-refundable.
